description: "Plan ONE increment of PHARN. Discovery-first, grounded in live state, pins the architecture content-hash, halts and asks before any build. Produces PLAN.md. FLOOR (deterministic, pharn/floor/check-plan-lessons.mjs): the PLAN must DECLARE `applied_lessons` — present, well-formed (`none` | `[L…]`), every cited id resolving to a real lesson heading, and every cited id REFERENCED in the plan body (sub-check D) so a citation costs a line — so a promoted lesson can never be silent
