Codex CLI has the largest configuration surface of the coding agents, and most of what goes wrong in it goes wrong in silence. On 17 October 2026, openai/codex had 129,639 stars and 23,030 open issues. A key with a typo is accepted unless you start Codex with `--strict-config`. A project's `.codex/` folder, with its config, hooks and rules, is ignored until you trust the project, and `codex exec` prints nothing about it. Ten keys are ignored in a project's `config.toml` with one warning. A rules file with an error stops `codex exec`, and the app server that the terminal UI is built on starts anyway and applies none of your rules. AGENTS.md is cut at 32 KiB without a word ([#13386](https://github.com/openai/codex/issues/13386), 12 reactions: "AGENTS.md is silently truncated and instructions near the end ignored"), an empty `AGENTS.override.md` hides the `AGENTS.md` next to it ([#54755](https://github.com/openai/codex/issues/54755)), a hook with a field Codex does not read keeps its 600 second timeout ([#50303](https://github.com/openai/codex/issues/50303)), a hook you changed is skipped until you trust it again, and an allow rule for `git` or `rm` is ignored.
