An agent skill that gives every working document an agent writes during a session - investigations, root-cause write-ups, analyses, plans, drafts, review notes - one home: `/_agent/docs/`. Never the repo's real docs, never a random file at the root, never lost with a worktree.

Each document carries a link to the session that produced it, so months later the reasoning behind every number is one `claude --resume` away. Folders that grow past a few documents get an index that says which one is current and which conclusions turned out wrong.

It uses the [Agent Skills](https://agentskills.io/home) format. Claude Code is the primary target; the folder convention works with any agent.

Sibling skill: [handoff](https://github.com/n0an/handoff-skill) keeps its step logs in `_agent/handoff/`, the same excluded folder.

What It Does

  • One folder for agent output: _agent/docs/ for documents, _agent/handoff/ for the handoff skill's step logs. Permanent documentation (docs/, README.md, ADRs) is a different thing and follows the repo's own rules.
  • Keeps _agent/ out of git through .git/info/exclude, so no tracked file changes and nothing shows up in git status. Resolves the main checkout from any worktree, so a document written in a throwaway worktree survives the worktree.
  • Session link in every header: date plus the session log path and the claude --resume line.
  • **Index past four docum
