When to use this skill

**TRIGGER (auto-invoke) when:**

Any /api/requests/* response has status=FAILED or error_message is set
A request has been PROCESSING for > 10 minutes with no progress
GET /health returns extension_connected: false
User reports any error string containing: `UNSAFE_
